How to Apply online for Central Government Health Scheme plastic card?

You can follow these steps to apply online for the Central Government Health Scheme (CGHS) plastic card:

1. Visit the official website of CGHS at https://cghs.nic.in/.
2. Click on the "Beneficiary Login" option available on the home page.
3. If you are a new user, click on the "Register Now" button and fill in your personal details such as name, date of birth, and mobile number to create your account.
4. If you already have an account, enter your User ID and Password to log in.
5. After logging in, click on the "Apply for Plastic Card" option available on the dashboard.
6. Fill in the required details such as your name, date of birth, and address, and upload your photograph and ID proof document.
7. Click on the "Generate OTP" button and enter the OTP sent to your registered mobile number.
8. Once the OTP is verified, select your nearest CGHS wellness center and click on the "Submit" button.
9. Pay the required fees online through net banking, credit/debit card, or any other available mode of payment.
10. After successful payment, you will receive a confirmation message on your registered mobile number and email ID.
11. Your plastic card will be printed and dispatched to your registered address within a few days.

Note: The CGHS plastic card is issued only to the CGHS beneficiaries who are entitled to avail the medical facilities under the CGHS scheme.
